Jacobi Medical Center and North Central Bronx Hospital recently celebrated their 2nd WeCOACH Graduation Ceremony on August 6. WeCOACH, which stands for Community Older Adults Care about their Health, is an evidence-based initiative designed to address chronic conditions among seniors over the age of 60 by enrolling them in a six week fitness program.
Trained peer coaches actively recruit patients from six HHC hospitals who have an A1c (blood sugar) level of 8.5 or above, which is considered uncontrolled. Patients are enrolled in a series of exercise classes and other activities hosted by senior centers run by the New York City Department for the Aging.
“We are very proud of our graduates and hope that they continue on their journey to a healthier lifestyle,” said Maria A. Mendoza, EdD, ANP, GNP-C, CDE, Network director of Chronic Disease Management. “I like to remind our patients that although they’re in the driver’s seat, our team is right behind them giving them the encouragement and support they need to succeed.”
The WeCOACH program has grown to include over 100 diabetic patients who have learned to enjoy the benefits of increased physical activity and improved socialization with others.
